Listed below are some common questions that our Case Manager receives on a daily basis:

Q:  My spouse is being discharged from the hospital/nursing home and I need help at home for my spouse, what can I do?

A:  All hospitals and skilled nursing facilities have Discharge Planners or Social Workers on staff. Their jobs are to meet with the patient, work with  the family, and create a safe discharge plan. The discharge plan can include in-home care if it is deemed medically necessary. They should work with the discharging physician and the insurance company to make sure the services ordered are covered. If the services ordered are not covered, they should inform the patient and family about what they will be responsible for.

Q:  I have an elderly neighbor who doesn’t seem to know how to take care of herself and is confused. Her children occasionally visit, but it seems that they are doing nothing to help their mother, and they may be spending her money on themselves. What can I do?

A: If you believe that there is neglect, abuse, or financial exploitation involved, you can directly make a report to Adult Protective Services (Adult Abuse 24 hour Hotline, Tel. # 1-877-767-2385). Another option is to get the client’s permission to call the Case Manager at Green Valley Assistance Services, # 625-5966. With consent given by the person in question the Case Manager will be able to assess the situation by making a home visit. The Case Manager will then determine if a report to Adult Protective Services is needed.

Q:  I am no longer able to take care of my spouse, where can I go to get help?

A:  There are several public assistance programs; however you must meet their eligibility requirements. Once qualified, these programs offer in-home care, respite services, and cover long-term care skilled nursing facility costs. You can directly apply for these programs or contact the Green Valley Assistance Services Case Manager, Tel. # 625-5966, who will assist you with this process. If you do not meet the eligibility requirements of the public assistance programs, the GVAS Case Manager will provide you with information on where you can obtain these services by paying privately.

Q:  I have doctor’s appointments in Green Valley and Tucson but I don’t drive, how can I obtain transportation assistance to get to my appointments?

A:  You can contact Friends in Deed, another volunteer group, churches, or pay Green Valley Gofers to take you to your doctor’s appointments. If you have insurance through the Arizona Health Care Cost Containment System (AHCCCS) with one of the four health Plans (APIPA, Health Choice, University Family Care, or Phoenix Health Plan), your health insurance will provide medically-necessary transportation to doctor’s appointments.  Please contact your insurance plan’s Member Services Department to make arrangements.

If you are already enrolled as a Case Management client with Green Valley Assistance Services, you may be eligible for a transportation voucher to get to your medical appointment (vouchers are provided on a limited basis).

Q:  Where can I get an application for food stamps or an application for the State’s health care program, AHCCCS?

A: Green Valley Assistance Services has applications for these programs. You can come to the office to pick up an application at: 250 E. Continental Rd., Ste. 102. Operating hours are Monday - Friday, 8:30 AM to 5:00 PM.
